The following forms are used by caregivers or the child's placement in a dependency or CPS case:
- Relative Information - This form can be used by any relative of a child in a dependency case to give the court information about the child.
- Confidential Information - Use this form if you do not want the other parties in the case to view the information provided in the Relative Information form (JV-285)
- Caregiver Information - This form can be used by the family or agency caring for the child to provide information to the court.
- Caregiver Information Instructions - Detailed instructions on who can fill out the Caregiver Information form and how to complete the form.
- Caregiver Information (Spanish) - The same Caregiver Information form as above, in Spanish.
